Volunteer Commitment Form

PARENT VOLUNTEER SERVICE FORM:

Upon completion of your volunteer commitment, fill out and submit the Parent/Volunteer Service Form in order to be refunded for your Volunteer Deposit.

This form needs to be filled out by the parent and signed by the team coach or Northwest Executive member to confirm that the parent has satisfied their volunteer requirements.

Once this form is fully completed, please email it to treasurer@northwestbasketball.ca to receive your refund.
